We are hiring a Geriatric Medicine Specialist who can manage complex health issues in older adults, focusing on multiple chronic conditions, and coordinating care with team in hospitals, clinics, or long-term care, emphasizing comprehensive assessments, personalized treatment, and promoting functional independence.
Key Responsibilities:
- Clinical Care: Diagnose and treat acute illnesses, chronic diseases (like heart disease, diabetes, osteoporosis), mobility issues, incontinence, and cognitive decline.
- Comprehensive Geriatric Assessment (CGA): Evaluate overall functioning, medications, social situation, and goals of care.
- Medication Management: Manage polypharmacy (multiple medications) and monitor for side effects and interactions.
- Care Coordination: Work closely with other healthcare professionals (nurses, therapists, PAs, social workers) to create holistic care plans.
- Patient/Family Education: Advise on lifestyle, nutrition, safety (e.g., driving, living alone), and advance care planning.
- Preventative Care: Offer advice on disease prevention and health promotion.
- Consultation: Provide expert opinions for hospitalized patients, often in emergency or acute settings.
- Setting Variety: Work in various environments like acute wards, rehab units, community clinics, care homes, and hospices.
Essential Skills & Knowledge:
- Expertise in atypical presentations of illness in older adults.
- Strong communication skills for complex family/patient discussions.
