Job Description

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and tasks may be assigned.

Perform psychosocial assessment, counseling, and referral of patients.

Recommend interventions appropriate to the findings of psychosocial assessment and reassessment.

Counsel patients and families relative to identified psychosocial issues or problems.

Recommend treatment changes based on psychosocial needs.

Provide a variety of casework and group work social services to patients and their families.

Identify social agencies and other resources as appropriate to the needs of the patients, including but not limited to financial or funding sources, such as, but not limited to, Kelana and private insurance.

Provide monthly reports to the Nursing Supervisor regarding financial issues/status of dialysis patients.

Provide rehabilitative and supportive social work services under the direction of an attending physician designed to restore patients to optimum levels of social health.

Maintain and submit accurate and timely clinical documentation for the patient's clinical record.

Advise and consult with family, physician, and other personnel about the social, emotional, and environmental needs related to the patient's health condition.

Develop a care plan in consultation with the physician and update and revise it according to the patient's response to treatment.

Assist patients with all appropriate medication assistance programs.

Conduct home visits as required by MOH regulations.

Respond to patients with empathy and positive interpersonal skills, promoting a positive mental attitude and a commitment to patients and employees.

Develop a psychosocial treatment plan to meet the social and emotional needs related to the health problems.

Document all communication and reports each visit.

Collaborate with the team to establish a care plan and long-term plan with revisions as needed.

Provide patient education as appropriate to the patient's needs.

Participate in patient care rounds or meetings on a regular basis.

Maintain confidentiality of all patient/family communication and interactions according to company policy.

Comply with Saudi regulations and MOH guidelines regarding social service and patient eligibility.

Promote a constructive approach to problem-solving within the facility and actively participate in the QAPI process.

Attend all staff meetings and in-services as they pertain to the job.

Perform KDQOLs as required.

Perform other duties and tasks as assigned.

QUALIFICATIONS REQUIREMENTS:

Education Requirements:

Bachelor's Degree in Social Work.

Experience Requirements:

Two (2) years experience in a social worker role, preferably in a dialysis or hospital setting.

Other Requirements:

Saudi Commission for Health Specialties Licensure is required.

Preferably BLS Certified from Saudi or American Heart Association.
