The Case Manager is responsible for coordinating patient care within mental health services while also supporting service growth through patient engagement, referral coordination, and stakeholder relationship management. This role ensures continuity of care, positive patient outcomes, and contributes to the visibility and utilization of mental health services.
Responsibilities
- Conduct comprehensive assessments of patients mental health, psychosocial needs, and support requirements to develop individualized care and discharge plans.
- Coordinate and monitor patient care across multidisciplinary teams, ensuring seamless communication between clinicians, patients, families, and external providers.
- Act as a key point of contact for patients and referral sources, supporting patient navigation, service understanding, and continuity of care.
- Support service growth initiatives by engaging with internal teams, referral partners, and community stakeholders to promote mental health services and pathways of care.
- Track patient progress, outcomes, and service utilization, maintaining accurate documentation and contributing data for operational, quality, and business review.
- Identify opportunities to improve patient access, experience, and service uptake, providing insights to leadership to support marketing, outreach, and business development initiatives.
- Facilitate patient acquisition and referral pathways through effective coordination with internal teams, external referral partners, and community stakeholders.
Qualifications
- Bachelor's degree in Nursing; Master's degree preferred.
- Minimum of 3-5 years experience in mental health services, case management, or care coordination.
- Exposure to referral management, patient engagement, or service development activities is an advantage.
